My mutation test came back positive for E453K.

 

Anyone know anything about this mutation and its relative sensitivities to the various TKIs?  I can't seem to find a lot online.

 

At this point, since my last test showed a 3.8 log reduction, we are doing a 6 week PCR [my regularly scheduled 3 month test]; however, I'm a bit more concerned that the 3.8 log reduction test was a fluke and the last one (2.9 to 2.7) might be correct.


Diagnosed with CML in July 2012 (33 years old)

MMR since March 2015; E453K mutation

600mg Gleevec

This article briefly talks about it. It looks like Tasigna may be able take care of it.

On page 36 it says the patients with that mutation reached MCyR on Tasigna and maintained it.

Now I have another question. 10 years or so ago I had my gallbladder removed. I had been having unexplained pain and terrible indigestion issues. Nothing in my diet could explain this other than it was hereditary. For years, I took Necium then I was switched to Prevacid. Even though I took a prescription antacid, I still took tums and Gaviscon daily. I was told by a friend to try drinking aloe Vera juice. The results were pretty amazing! Within one month, I reduced the Prevacid dosage and a few months later, I stopped taking it all together. I have continued this regimen of drinking aloe Vera juice daily and rarely have any GI issues at all. I've seen many people comment that Gkeevec causes nausea and or stomach issues, yet I've never experienced any side effects. I'm wondering if it's the aloe juice. My question though is could the aloe juice cause anything negative effects like cause the Gleevec not to absorb or be as effective. My doctor is aware that I drink it and is ok with it.
Anyone can respond! Just looking for some insight! Thanks!

I have heard that Aloe juice is good for Gerd. How much do you drink on average per day. I have never tried it.

I drink only 8oz per day. I've bought really expensive brands and lesser expensive and honestly I saw no difference. I used to buy Fruit of the Earth brand from walmart or walgreens and now I buy HEB's store brand.

What a coincidence I had my gall  bladder yanked out yesterday.  I'll have to try this juice.

Tom,

That E453K kinase mutation is far from the areas where the drugs work.  So I doubt it impacts the efficacy of any of the TKI drugs.

Thanks, Trey!  The lab that reported the test result said that "E453K has been reported to confer resistance to BCR-ABL1
tyrosine kinase inhibitors."  My doctors have said there's not a lot of firm data because it is a relatively rare mutation, but it has been correlated with imatinib resistance.

From what I can determine E453K is only a problem when it is associated with other kinase mutations.  But alone it seems benign.  Gleevec works in the kinase P-Loop, far away from the A-Loop where E453K occurs.  So I am very skeptical about E453K being a problem in isolation.
