Does anyone know if there is collateral damage to one's system by starting CML with higher WBCs than lower ones?

Just based on the white count alone, no.  The counts are not as important as the phase of the disease and the quality of the bone marrow.

I suppose you can say there can be damage with high WBC counts.  As some have pointed out, once their levels return to normal the damage seems to heal and reverse.  At the start, my counts were in the 360K+ range with 1.2 Mil Platelets.  My vision turned green in the morning and lasted longer and longer.  Then I developed a permanent tiny blind spot in my right eye.  The green tint to my vision has long repaired itself and my elevated createnine levels have finally dropped into normal range now that i'm no longer taking Gleevec.

As threedprof said, there can be eyes problems. Apart from that I don't think there can be something else. I had 400k white cells, and it decreased quite well with hydrea and then sprycel.

Possibly some permanent hearing loss from high WBC (459k) and/or from Gleevec, not sure which, it was very noticeable by a couple of months after dx. I did some online searching and found a couple of similar cases of sudden hearing loss soon after starting G, but with higher WBC (600-700k), and also that Gleevec was being checked out by the FDA due to the number of reports re hearing loss. We may never know all the consequences due to the high WBC, since we may not associate cause and eventual effect.

Be alert, but not overly concerned.

• Dx Oct. 22, 2008, WBC 459k, in ICU for 2 days + in hospital 1 week

• Leukapheresis for 1 week, to reduce WBC (wasn't given Hydroxyurea)

• Oct. 28, 2008: CML confirmed, start Gleevec 400mg

• Oct. 31, 2008: sent home when WBC reached 121k

• On/off, reduced dose Gleevec for 7 months

• April 2009: Started Sprycel 100mg

• Sept. 2009: PCRU 0.000

• Sept. 2011: after 2 years steady PCRU & taking Sprycel 100mg before bed, quit Sprycel (with permission)

• Currently: still steady PCRU, testing every 6 months 🤗

— Fatigue, hearing loss continue, alas, but I prefer to think it is all getting better!
