I've been on Gleevec since the third week of September, finished my 90 day supply about a week after my test on December 18th.  

 

From my Oncologist:  

 

The PCR demonstrated a reduction for the BCR-ABL fusion protein to 1.6% from 66.4%.  This is a reduction of at least one log which is great."  

 

Told him that was my best Christmas present yet this year.   :)

Closer to a two log drop ... 1.6 log drop to be more precise. Congratulations! And in three months time. You're responding to Gleevec very well. I am surprised that they either don't do FISH testing or are not telling you the results. At 66% PCR your FISH would certainly be positive (actual CML cytogenetic observation). But at 1.6% PCR, chances are your FISH would be zero or very close to it. Perhaps FISH testing is going the way of the Dodo bird.

 

Your next 3 month PCR will very likely put you below 1.0% perhaps much lower. Your new target to reach is 0.1% (major molecular remission or MMR) - another more than a log drop.


Diagnosed 11 May 2011 (100% FiSH, 155% PCR)

with b2a2 BCR-ABL fusion transcript coding for the 210kDa BCR-ABL protein

 

Sprycel: 20 mg per day - taken at lights out with Quercetin and/or Magnesium Taurate

6-8 grams Curcumin C3 complex.

 

2015 PCR: < 0.01% (M.D. Anderson scale)

2016 PCR: < 0.01% (M.D. Anderson scale) 

March        2017 PCR:     0.01% (M.D. Anderson scale)

June          2017 PCR:     "undetected"

September 2017 PCR:     "undetected"

At the earliest opportunity, and whenever possible, lower your TKI dosage; TKIs are toxic drugs and the less we take longterm the better off we are going to be ... this is especially true for older adults.  

 

In hindsight I should have started my dosage reduction two years earlier; it might have helped minimize some of the longterm cumulative toxic effects of TKIs that I am beset with.  

 

longterm side-effects Peripheral Artery Disease - legs (it's a bitch); continuing shoulder problems, right elbow inflammation.   GFR and creatinine vastly improved after stopping Gleevec.

 

Cumulative Gleevec dosage estimated at 830 grams

 

Taking Gleevec 400mg an hour after my largest meal of the day helped eliminate the nausea that Gleevec is notorious for.

Dx  April 2013, FISH 62,  BMB not enough for PCR test; put on Gleevec 400;

 August 2013, FISH 8.7;

Oct 2013, FISH 5.6

Stopped Gleevec Nov 2013 for 6 weeks due to terrible side effects; Jan 2014 started Sprycel 50mg;

Feb, 2014 PCR  6.8

May,2014  PCR   .149

Aug, 2014 PCR    .015

Nov. 2014 PCRU

March, 2016  went down to 40mg Sprycel

Oct. 2016   stopped Sprycel for a couple weeks due to concern about shortness of breath.  Echo showed mild PAH.

Nov 1 2016  resumed Sprycel 20 mg daily 

Dec 2016  PCRU

March 2017  PCR 0.020

May 2017     PCRU

I've had a very good response to gleevec. Fish was 92%. First pcr was done at 3 months after starting med and it was 32%. 6 months later I was 0.88%. Next test this week. Since I don't know my initial pcr since they didn't do it for the first 3 months, how do I figure log reduction?

Gail, I would ask your Oncologist for the results, they should have them all from your first test at diagnosis, I just can't see them not having done one then as it is required to get the diagnosis correct.

No pcr, went straight to BMB and fish. I had to ask for pcr after learning I needed it from this site. Already discussed it with doc.
