Question about Sprycel and Synthroid

Hi all

 

Was wondering if anybody takes Sprycel and Synthroid/levothyroxin (for hypothyroid), and if so, has your synthroid or levothyroxin dosage needed to increase since starting sprycel?  From what I can tell Sprycel (100 mg) seems to interfere with the synthroid/levo.  Since I was dx with cml in july 2012, my dosage of synthroid has gone up in the last couple years, from 25 or 50 ug initially to my present dose of 125 ug.  It is now stable so hopefully will not need to go up further.

I am on Sprycel 60 mg and Synthroid 50 Mcg. I am not on the generic Synthroid. I was dx w CML in late February 2015, and I have not had any drug interaction nor an increase in the Synthroid medication. My levels seem to be the same as before dx. I have heard of others having a different reaction and their needing to up their dose, but that is not the case with me.

Dx 2/2015 BCR-ABL1 (p210) 85.2% (IS) 3/15

     22%  5/15     0.13% 6/15   PCRU attained 9/15

Initial dose Sprycel 100 mg 3/15  Lowered 80 mg 5/15   Lowered 50 mg 1/16

Note: dose lowered bc of side effects - not bc onc wanted to reduce dosage

Sprycel: Currently 50 mg per day - taken 10 pm

I was on Sprycel when I needed to start Synthroid (I do not take generic either). I had a total thyroidectomy two months after starting Sprycel 100mg. I take (2) 112mg of Synthroid and I also take Cytomel (straight t3) 5mg. I do separate them out from one another. I take my synthroid and cytomel in the morning and then Sprycel at 4pm. It took them a long time to find the dose that would keep me suppressed enough (they keep my level at .2 since I had thyroid cancer, so, I teeter on the edge of hyperthyroid), but I think that's common for it to take months to find someone's correct dose after a total thyroidectomy. My thyroid levels have stabilized in the last three months.


7/2014 Diagnosed,8/14 Started 100mg Sprycel, 9/14 Thyroidectomy (thyroid cancer)

8/2015 Undetectable, 12/15 Plural Effusion (3 wk drug break)

1/2016 Started 70mg Sprycel, 3/16 Plural Effusion (4 wk drug break)

3/16 .014 after a wk w/o meds

4/16 Started 400mg Gleevec

4/16 Undetectable, 7/16 Undetectable, 10/16 Undetectable, 2/17 Undetectable, 5/17 Undetectable, 8/17 Undetectable

I've been taking levothyroxine since a subtotal thyroidectomy in 1979, both Synthroid and in later years the generic.  My levels are tested periodically and have not changed since 2009 when I started Gleevec and then Sprycel (last four years.)  Therefore, the levothyroxine dose has not changed either.


Dx July 2009 on routine physical.  WBC 94.  Started Gleevec 400 mg Sept 2009.  MMR at 2yrs.  Side effects (malaise, depression/anxiety, fatigue, nausea, periorbital edema) never improved.  Kidney issues developed because of Gleevec.  Switched to Sprycel 70 mg in Aug 2011.  Above side effects disappeared or improved.  Have been MR3.5 - 4.5 ever since.  Two untreated pleural effusions followed by one treated by stopping Sprycel Jan 2017.  After 9 weeks, PCR showed loss of MMR; re-started Sprycel at 50 mg and in 3 months was back to <0.01% IS.  Pleural effusion returned within a couple of months, same as before (moderate, left side only).  Stopped Sprycel 50 mg for 12 weeks; pleural effusion resolved.  At about a monthoff the drug, PCR was 0.03; at 11 weeks it was 2.06 - lost CCyR? Have returned to 50 mg Sprycel for 3 weeks, intending to reduce to 20 mg going forward.
