With pancreatitis, there is usually severe abdominal in the left upper abdominal quadrant that can radiated to the back, left flank, or left shoulder, and is usually continuous. OTH, gall bladder pain is in the right upper quadrant and can radiate to the right shoulder. IIRC, pain from GERD can also radiated to the back.

If the pain is much worse after a high fat snack or meal, it could be your gall bladder.
But it could be pancreatitis. Or something else. Check with your doc and have tests run within a day of the pain. Some tests include serum amylase and serum lipase tests, as well as ALT/AST.
