45 days on Sprycel and PE

I am newly diagnosed and in treatment with 400 mg Sprycel for CML. Forgive me, I don't know all of the terminology or shorthand I have been reading, but bottom line is my white count was over 280K at diagnosis. They started me on allopurinol (which I am allergic to) and hydroxyurea. I was very glad to start the Sprycel and have seen steady improvement in platelets and white count. Although my Hgb is 10.5, I have been happy with my this progress....but now after complaining of burning pain in the scapular area and a chest xray...it looks like I have a PE and I have been ordered off Sprycel for 5 days.

I am worried the PE will mean they take me off the Sprycel and try something else - when it seems that TKI has been doing a very good job. Is there any way to avoid PEs and have you ever heard of someone getting changed right away or do they wait. Does anyone else have experience with this?

I guess I am just worried. Thanks for any advice. Deborah

Sorry to hear about your situation.  I'm a little confused not sure if it is a typo on the dose or the drug but I don't believe you are taking 400mg of Sprycel.  400mg is the daily dose for Gleevec and standard dose for Sprycel is 100mg.  Please clarify.

The incidence of PE with Sprycel is higher than we would like.  There are various options.  Sometimes a break will resolve and restart without issue.  Sometimes we modify dose and see how it goes.  Given how quickly this happened I would be inclined to say this probably just isn't the right drug for you.  Fortunately there are options with cml treatment and we have other drugs.  No reason to think any of them wouldn't work well for you .  So discuss with your dr and if in doubt get a second opinion.  The important thing to know is that this isn't that uncommon and it shouldn't have any impact on your long term.  You just need to find the right drug for you.

OMG...sorry that IS a typo, I am on 100 mg Sprycel - at least for now.

Thanks for the quick response...guess it's not going to be as simple as I had hoped.

Good to know there may be other options.

That make sense.  It sucks to have a bumpy start  but again not uncommon and not problematic if it is caught and drug is stopped.  We are fortunate to have options, it's a bump in a bumpy road but try to let it get you down.  Like I said a switch to another drug and you will likely do fine.  Good luck

Some people who have PE on 100mg, avoid it on a dose of 50mg.  Also, a diuretic may be helpful.

Hi, Deb,

I also had a rocky start in 2009. After 8 months on Gleevec my liver took extreme exception to the drug. I was switched to Sprycel and I had some lovely side effects to work through but after 6 months or so things settled down. I developed PE at the 2.5 year mark - something I don't really want to ever go through again. My dosage was reduced to 50 mg. (my docs were really good at helping me with this because they knew I really didn't want to try and acclimate to yet another drug after all I went through with Sprycel).

Two years later (knock wood) I'm back to MMR.

I don't know if there is a difference between those of us who develop PE right out of the gate vs. those of us who develop it 2+ years later in terms of being able to successfully make modifications to the dosage we take. I do believe you need to take the time to let the PE fully resolve itself before making any decision about your meds.
